    IF YOUR IDENTITY HAS BEEN MISUSED TO ESTABLISH SERVICE WITH DISCOUNT POWER, TAKE THE FOLLOWING STEPS:

    1. Contact us immediately at 1-877-455-4674 from 7 a.m. to 10 p.m. C.T.
    2. Determine if your identity was used to open an account with us.
    3. Fill out an ID Theft Affidavit and have it notarized.
    4. File a police report and get a complete copy.
    5. Gather all required documents and send them to Discount Power.

    REQUIRED DOCUMENTS TO DISPUTE AN ACCOUNT

    Send us the following documents so we can start our investigation:
    1. Copies of 2 types of utility bills or formal correspondence in your name from other providers
    2. A notarized ID Theft Affidavit, stating that you’re willing to press charges if necessary
    3. A complete copy of a full police report
    4. A valid government-issued photo ID card (for example, your driver's license, state-issued ID card or a passport)
    5. A legible copy of your Social Security card, upon request, to verify your identity
